Jeff Davis Hospital is now offering our patients easy and private access to their medical information online, so you can view your personal health record whenever and wherever you have access to the internet!

You can gain access to the patient portal by either providing Jeff Davis Hospital your personal email address on registration or you can follow the "create an account" link and self register.

Jeff Davis Hospital encourages you to periodically review your medical information available through this portal. Not only will this provide you the opportunity to access useful information about your current health conditions and/or test results, but it will also enable you to verify the information in your record.

Medical Identity Theft is a serious issue. Issues such as unemployment, loss of insurance benefits, the opioid epidemic, and other social factors have driven an increase in such theft. A thief may use your name or health insurance numbers to see a doctor, get prescription drugs, file claims with your insurance provider, or get other care. If the thief's health information is mixed with yours, your treatment, insurance and payment records, and credit report may be affected.

As you routinely review your records, if you see signs of medical identity theft, most notably documentation for care you did not receive, please notify the Jeff Davis Hospital HIPAA Privacy Officer immediately by calling 912-699-6218 Then, visit IdentityTheft.gov to report and recover from identity theft.

Note: Other signs of medical identity theft include:

• A bill for medical services you didn't receive
• A call from a debt collector about a medical debt you
don't owe
• Medical collection notices on your credit report that you
don't recognize
• A notice from your health plan saying you reached your
benefit limit
• A denial of insurance because your medical records show a
condition you don't have.

If any of these circumstances happen to you, please notify the HIPAA Privacy Officer.
